 Program: Fun & Faith-Filled JOYFUL STEPS Toddler Dance Class (Ages 2–5) 

Organization: M.O.M. Nonprofit LLC 

Description: Our Fun & Faith-Filled JOYFUL STEPS Toddler Dance Class is a summer intensive designed for toddlers ages 2–5, introducing the beauty of liturgical dance and the energy of Christian hip hop in a loving, joyful environment. Classes are held twice per week with a suggested minimum donation of $5 per class to support the program.

Waiver and Release of Liability

I, the undersigned parent or legal guardian of the child named below, hereby grant permission for my child to participate in the Fun & Faith-Filled JOYFUL STEPS Toddler Dance Class offered by M.O.M. Nonprofit LLC and ITS AFFILIATES. I understand that this program involves physical activities, including dance and movement, designed for toddlers ages 2–5.

Assumption of Risk: I acknowledge that participation in the dance class involves inherent risks, including but not limited to falls, collisions, or other accidents that may result in minor or serious injury. I understand that while M.O.M. Nonprofit LLC and ITS AFFILIATES take reasonable precautions to ensure safety, these risks cannot be entirely eliminated.

Release of Liability: In consideration of my child’s participation, I hereby release, waive, discharge, and agree not to hold M.O.M. Nonprofit LLC and ITS AFFILIATES, its instructors, staff, volunteers, or affiliates liable for any injuries, damages, or losses sustained by my child during or as a result of participating in the Fun & Faith-Filled JOYFUL STEPS Toddler Dance Class, except in cases of gross negligence or intentional misconduct.

Health and Safety: I confirm that my child is physically capable of participating in the dance class and has no medical conditions that would prevent safe participation. I agree to inform M.O.M. Nonprofit LLC and ITS AFFILIATES of any health concerns or special needs that may affect my child’s participation.

Donation Acknowledgment: I understand that a minimum donation of $5 per class is suggested to support the program, and I agree to comply with this contribution for my child’s participation.

Photo/Video Consent: I grant M.O.M. Nonprofit LLC and ITS AFFILIATES permission to use photographs or videos of my child taken during the class for promotional purposes, such as on social media or in printed materials, unless I provide written notice otherwise.

Emergency Authorization: In the event of an emergency, I authorize M.O.M. Nonprofit LLC and ITS AFFILIATES to seek necessary medical treatment for my child if I cannot be reached.

By signing below, I acknowledge that I have read, understood, and agree to the terms of this waiver and release of liability.
